It's very simple.

You just have to use the following rule :

If the question needs a question word, follow this order QASI

Question Word

Auxilary

Subject

Infinitive (and later, the rest of the question)

If the question is answered with yes / no, follow this order ASI :

Auxilary

Subject

Infinitive

(and later, the rest of the question)

If the main verb is the verb to be, it takes the place of the auxilary and there's nothing in the place of the infinitive.
